Reverse Seroconversion of Hepatitis B virus after High Dose Chemotherapy with Autologous Peripheral Blood Stem Cell Transplantation in a Patient with Multiple Myeloma

Abstract
Hepatitis B virus (HBV) reactivation with acute hepatitis B in patients previously positive for hepatitis B surface antibody (Anti-HBs Ab), so called reverse seroconversion, has been considered to be a rare complication after autologous hematopoietic stem cell transplantation (auto-HSCT). We experienced a case of reverse seroconversion in a patient with multiple myeloma after auto-HSCT, so we report our case with reviews of literatures. A 42-year-old woman was diagnosed as multiple myeloma and she underwent high dose chemotherapy with auto-HSCT. The laboratory findings for HBV infection before therapy were anti-HBs Ab positive, HBs Ag negative and normal liver function test. However, HBV reactivation with acute hepatitis B developed after high dose chemotherapy with auto-HSCT. She has taken an antiviral agent, clevudine and her hepatitis B was recovered. The reverse seroconversion with acute hepatitis B can be developed in patients underwent high dose chemotherapy with auto HSCT. Therefore, we should carefully evaluated HBV markers in such patients and further studies for prophylactic management are warranted.
